The Myocarditis Foundation, in collaboration with researchers from the Mayo Clinic in Rochester, Minnesota, and the Mayo Clinic in Jacksonville, Florida,  is conducting a study to identify risk factors for anxiety and depression in people with myocarditis and their caregivers. The emotional impact of myocarditis has received only limited attention by researchers. The results of this study will be an important addition to our understanding of the disease. The study is an electronic (internet) survey that will ask several questions related to demographics, general heath, stress, anxiety and depression. Your responses are confidential. He will obtain consent from the patient, collect their email and have the survey center send the survey link.

You will then be able to start the survey after providing your consent. The survey takes about 45 minutes to complete. Some people may need more time than that. But you can take breaks by saving your progress and then logging back into the survey using a password that will be provided to you. You will not receive compensation from study participation.
